We will gladly file claims on your behalf for all insurance companies and accept assignment of benefits, to enable and assist you in maximizing your benefits. Whenever possible and applicable, we will also submit claims with your medical carrier on your behalf, to again assist you in defraying the costs of your treatment. The patient is, however, responsible for the cost of the treatment. Payment for services rendered is kindly requested at the time of treatment.
Please remember that “insurance” is a benefit offered to you by your employer. It is considered a method of partially reimbursing the patient for fees paid to the doctor and is not a substitute for payment. Most dental insurances have a “fixed maximum annual benefit” of varying amounts per year, with additional stipulations. It does not have to be on a calendar year basis. Unlike “traditional” medical insurance, “dental insurance” is more an “assistance” program, rather than a traditional insurance. Some companies pay fixed allowances for certain procedures, while others pay a percentage of the charge. Please understand, it is your responsibility to pay any deductible amount, estimated co-insurance and any other balance(s) not paid for by your insurance company.
We will do our best to answer any questions that you may have about your dental insurance and to assist you in maximizing your insurance benefits. Since we file your claims electronically, “most” insurance companies will respond within 4 to 6 weeks. However, PLEASE understand that you are ultimately responsible for your account. Please remember, you are also fully responsible for all fees charged by this office within 90 days of the procedure, regardless of your insurance coverage.

Please understand, our office reserves all appointments specific to each patient’s dental needs. If you must change an appointment, please provide us at least 2 working days (48 hours) advanced notice, so that we may use the time to accommodate other patients. Broken appointments within 48 hours will be subject to a fee. Broken and missed appointments, create scheduling problems for both, other patients and our practice. We value your time and kindly request that you please value ours. We are not a high-volume practice, and the time “reserved” for your appointment, has been “reserved exclusively” for you.
